Submitting FORM A To The Registrar Office

Submission of Form A along with the documents mentioned above to the Registrar Office (that office where your birth is recorded) should mark the end of your name Change Procedure in Goa. The Form A that you submit to the Registrar Office, shall undergo an enquiry conducted by the registrar and shall issue an order within sixty days, based on the result of the review of your application. If the result is positive, the rest, i.e., publication of private advertisement in the Goa Gazette is taken care of by the Registrar office.

Received An Objection For Name Change?

If you are one to have received an objection after submitting your Form A to the District Registrar Office, there is a provision where you can reappeal the District Magistrate Office seeking the cause of rejection of your Name Change Form and ways to rectify the same. Within thirty days from the date you receive the order of rejection of the order or Form, appeal to the Chief Registrar requesting Change of Name. If no objection is received the Registrar shall under the extract of Section 17 of the Registration of Births and Deaths Act, 1969 based on the applicant’s birth registration, enters the relevant details in the registrar of his/her birth which can either be the changed name or surname.
